Check with your insurance to verify eligibility:

Confirm your active coverage of benefits- specifically for behavioral health.

Confirm Haelen Mental Health is part of your in network plan.

Appointment costs are negotiated with each insurance company. If you have a high deductible plan, the appointment costs will be processed by your insurance company and patient responsibility is calculated by the insurer. Your insurance will send a bill to you directly until your deductible or out-of-pocket max is met. Our practice will bill your co-pay the day of service for each visit if that is applicable.

Super-bills may be provided for out-of-network claim submission; this is not an option for Medicare and Medicaid.​

As a practice we do not complete short term disability paperwork or conduct court ordered evaluations.
Payments & Pricing

All co-pays, co-insurance and deductibles are the responsibility of patients at the time of service. Those clients with insurance deemed out of network are responsible for paying self-pay costs and a superbill will be provided.

A valid credit card should be kept on file. We accept all major credit cards and HSA cards.

Late Cancellation and No Show

Appointments cancelled or rescheduled within 24-hours of the appointment time will be charged a no show fee, which is the self-pay cost of the designated appointment.

Please note that insurance does not cover no show fees and it will be charged to the credit card on file.
