Tara Ostromencki, DNP, RN, PMHNP-BC

Tara is a proud Wisconsin native who has spent her life along the Driftless and Bluff Coast regions. As an ANCC Board-Certified Psychiatric Mental Health Nurse Practitioner, she is deeply committed to giving back to the communities that have shaped her. Licensed to treat individuals across the lifespan, she brings extensive experience in assessing and managing a wide range of mental health concerns.  Tara’s journey into behavioral medicine began early. Growing up with a parent who lived with severe mental illness gave her a profound sense of empathy, resilience, and an unwavering belief in the power of community, family, and friendship to support mental wellness.

Her approach to care is both integrative and evidence-based.  Tara believes in the essential connection between mind and body in healing, and tailors treatment plans to reflect this philosophy. Depending on individual needs, care may include up-to-date psychiatric medication, natural treatments such as supplements, nutrition and exercise recommendations, and stress management strategies.

Each treatment plan is personalized, collaborative, and designed to empower clients in a supportive, stigma-free environment. During the initial visit, clients can expect a comprehensive psychiatric evaluation, thoughtful discussion of their treatment goals, and clear education about their diagnosis and available options.

 

Tara sees individuals aged 6+ via Telehealth and in person in the Wisconsin office, located in Lacrosse.
